Request a SETAC Information Portal User ID

Please use this form to request access to one or both of the SETAC Information Portals. You will need to specify which contract. Each contract has a SEPARATE website name and URL.

Once this request is submitted, an email is generated and sent to the DMD Server administrator. Once a userid and password have been established, an email will be sent to the new SIP user, using the email address provided on this page.
